In 2003, PhyNet Inc., a Texas nonprofit healthcare organization, was established as a physician’s network of primary care, pediatric care, internal medicine and occupational medicine health clinics. Operating in eight East Texas communities and one in San Antonio, Texas, our physicians and advanced nurse practitioners provide quality primary care and coordinated care services. Positioned to Transform Healthcare
Since the Affordable Care Act (ACA) was passed in 2012, healthcare has turned its focus to health management and monitoring as an answer to sustainability. The Centers for Medicare and Medicaid Services (CMS) is spearheading “population health,” and for good reason. Population health is composed of communities of children, adults and senior citizens of all ethnicities. Until population health became a focus – and even now – healthcare systems treated patients all the same: When individuals got sick, they were provided with a medical assessment and medications, and then went unmonitored until a future health problem developed.
CareTeams are Health Advocates
When patients enroll in the PhyNet CCM (Chronic Care Management) program, we provide clinical evaluations to diagnose and identify “at risk diseases.” The data is powerful knowledge our CareTeams use to customize care plans designed for prevention. Early detection is the best prevention. Care plans include wellness visits, check-ins by phone, education and coaching for a more proactive approach to achieving better outcomes. The CCM program is designed for senior adults, as their need to stay more health-conscious is especially important. The PhyNet CareTeam is Your Health Advocate – bridging gaps in care and providing the extra attention you need to stay healthier and live better.

Years ago, Thomas Edison predicted the future of medicine: “The doctor of the future will give no medicine, but will interest his patients in the care of the human frame, in diet and in the course and prevention of disease.” Who knows your body better than you? At PhyNet we know it’s you – the patient. To stay better informed, patients need better access to the information. The ongoing job of our CareTeam is to build and maintain medical histories on patients for meaningful use goals. (Meaningful Use is using certified electronic health record (EHR) technology to: Improve quality, safety, efficiency, and reduce health disparities • Engage patients and family • Improve care coordination, and population and public health • Maintain privacy and security of patient health information.)
Open Systems Empower Patients
PhyNet CareTeams’ extended services include navigation to needed medical specialists, including hospital stays and rehabilitations. Not relying on our own medical network, PhyNet utilizes an “open system” of practice, referring patients to any and all specialists, labs, hospitals and rehabs. As a result, the patient is not pigeonholed to in-network referrals and subject to fees with no choice. Second opinions and fee-for-service analysis are almost unheard of in the healthcare industry today. However, at PhyNet, we feel this transparency is what best demonstrates what an Accountable Care Organization should
be and what patients need to make informed choices. (Fee-for-service (FFS) is a payment model where services are unbundled and paid for separately. In health care, it gives an incentive for physicians to provide more treatments because payment is dependent on the quantity of care, rather than quality of care.)
Expanding the Boundaries of Outpatient Care
PhyNet is part of a network of affiliated healthcare organizations owned and managed by the same group. PhyNet evolved from a management services organization. Responding to physician demand, PhyNet Inc. formed as a closely held corporation of its parent company, Triad Physician Resources, Inc., and became Triad’s sole client as we realized the advantages of bringing physicians into one organization. PhyNet continues to expand to become more relevant to payers and bring value and cost efficiency to the marketplace using digital and virtual technology, patient engagement programs and interoperable service lines.
Triad Physician Resources, Inc. (TPR) is a healthcare risk management partner and practice management organization that provides the management services to operate PhyNet and Prime Care Managers. We have more than 50 years of experience building successful healthcare practices and know the power that independent primary care physicians have to help our communities thrive. Formed in the year 2000, we have evolved to support primary care practices and initiatives, providing financial, IT, credentialing, contracting and other practice management services. TPR provides integrative management support for other outpatient entities, including home health, outpatient rehabilitation and chronic care organizations.
PrimeCare Home Health care teams focus on our patient’s total well-being, including prevention and education to avoid emergent situations and unplanned re-hospitalizations. Our PrimeCare Home Health coordination works to ensure smooth transitions from hospital stays while providing navigation to specialists that are the optimum fit for the patient. PrimeCare Home Health staff are licensed nurses, comprised of Skilled Nursing and Care Coordinators that practice in Texas with a wide variety of specializations, including but not limited to Transitional Care Management, Physical Therapy, Chronic Care Management, Medication Management, Wound Care, and 24/7 On-Call Care Team.
