Family Nurse Practitioner to serve as primary and specialty health care provider to work with patients throughout their lives, diagnosing illness, conducting exams, and prescribing medication.

We are looking for a professional Nurse Practitioner who can deliver primary medical care to a wide variety of patients. The Nurse Practitioner will be responsible for caring for patients, maintaining accurate and current patient records and scheduling and administering follow-up appointments to patients as required. The successful candidate will work as a team with our nurses and physicians and will assist in delivering premium care to each and every patient.

Medical Assistants of various levels of clinical experience are needed. Primary work day include: working closely with CareTeam and patients assisting with all medical follow-through, including: care coordination, patient engagement, chronic care enrollment, billing and meaningful use recording / EHR medical health record management. CareTeam environment assisting Medical professionals, MD, DO and FNPs.
